Plan treatment as ongoing for as long as impairment lasts, not a time-limited childhood course, and weigh that within-person, on-med months reduce serious life risks, not just symptoms.

Stimulant and non-stimulant medication for ADHD is non-curative: it temporarily manages symptoms while it is active, and behaviour reverts to the pre-treatment baseline once it washes out. This is by design, not a sign the medication has failed — the longer it is taken the longer its benefits last, and stopping even for a month returns symptoms to where they started. A useful framing is the ‘diabetes of psychiatry’: you would not expect lasting benefit from treating diabetes or epilepsy only in childhood and then stopping, and ADHD is the same kind of persistent condition that needs ongoing management.
The implication is to keep treatment continuous through the high-risk adolescent years and into adulthood when impairment persists, rather than discontinuing once things improve — childhood-only treatment shows no detectable effect on adult outcomes. Treatment can still flex with demand: during genuinely low-demand periods (a change of school, teacher or job) some people may not need medication for a while, but they should expect to return to the full package when demands and symptoms rise again, since impairment is the interaction of symptoms with what life is asking. When weighing the decision, the relevant comparison is broad: within-person studies find that during on-medication months, risks of accidents, injuries, suicide and early mortality (alongside driving, work, education and peer outcomes) all drop versus off-medication months.
