The stimulant paradox in ADHD

Background

In ADHD the prefrontal pathways are short on dopamine, so the brain runs under-aroused and restless. A stimulant — prescribed medication, but also caffeine, amphetamine or cocaine — tops up that dopamine, and the felt effect is paradoxical: rather than a euphoric buzz, many people with ADHD report calm, slowing of a racing mind, and the ability to ‘bring their thoughts together’ while everyone around them is climbing the walls. One common description is that it ‘switched off the tumble dryer in my head and let me concentrate’. This has two consequences. First, it is a powerful driver of self-medication and addiction: substances that calm rather than excite an ADHD brain are filling a real dopamine gap, not merely chased for recreation — which is part of why ADHD and substance addiction are tightly linked. Second, it offers a rough clinical clue: when someone reports that stimulants have ‘no effect’ or actively calm and clear them rather than producing a high, that paradoxical response points toward ADHD (one clinician reports this holding in roughly 70% of assessments). It is a clue, not a diagnosis, and it should never be used to encourage illicit use.
